          What Is A "508-R Form" ?
 

Anyone convicted in NC of DWI is required, among other penalties, to obtain a substance abuse assessment and complete the level of DWI program required by that assessment, per NC statutes governing DWI programs. Program completion must then be proved to the NC DMV.

The 508-R Form is the official document required by the NC DMV to prove that a person convicted of a DWI in NC has completed a DWI program that meets the guidelines set by the State of NC. When a 508-R Form is submitted on your behalf, it's a statement of confirmation by a NC-licensed DWI agency that the program you completed was properly licensed and that it met or exceeded the specific NC guidelines that govern DWI programs.

The 508-R Form may only be filled out by a NC-licensed DWI agency located in NC.

The NC DMV will not accept program completion documents directly from you or from the agency that provided your program outside NC. If you have a NC DWI but wish to complete your DWI program in another state, NC state guidelines require you to hire a NC-licensed DWI agency that offers Out Of State Case Reviews.

There are 50 US states, of course, and each has its own set of guidelines as to what constitutes a DWI or DUI program. They are all different. The standard DWI / DUI program in your state may not be accepted by North Carolina.

It often happens that a person with a NC DWI will take a DWI program in another state without understanding the NC guidelines and requirements only to find out the program they've already completed will not be accepted by the NC DMV. This is why it is important to consult with a NC-licensed DWI agency prior to enrolling in a program in your home state to ensure you complete a DWI program that satisfies NC requirements and standards.

�stop� that is entered on a convicted DWI offender�s driving record by the Division of
Motor Vehicles.
Upon completion of required services, an authorized DWI provider forwards the
Certificate of Completion (508-R) form to the Division of Mental Health, Developmental
Disabilities and Substance Abuse Services (DMHDDSAS). It is reviewed, by the State
DWI Services Office, for accuracy, completeness and compliance with State statutes
and administrative rules. An original is forwarded to the Division of Motor Vehicles for
further processing


"The DMH 508-R form was originated in 1987 and revised effective January 1, 1996. This form is used to verify completion of DWI substance abuse services and facilitate the removal of the "stop" that is entered on a convicted DWI offender's driving record by the NC Division of Motor Vehicles.
